These Terms of Service ("Terms") govern your access to and use of Customer Account Forms("the App"). By installing or using the App, you agree to these Terms.
1. The service
The App provides tools for merchants to create forms on customer account and order pages, manage submissions, and export responses. Features may change over time as we improve the service.
2. Eligibility
You must be authorized to act on behalf of the store that installs the App and must comply with all applicable laws and platform policies.
3. Subscriptions and billing
Paid plans are billed through the platform's app billing system. Fees, trial periods, and renewal terms are shown during installation. You may cancel according to the platform's subscription management tools.
4. Merchant responsibilities
- You are responsible for the forms you publish and the data you collect.
- You must provide accurate privacy disclosures to your customers.
- You must not use the App for unlawful, deceptive, or abusive purposes.
- You must keep your account credentials and app permissions secure.
5. Acceptable use
You agree not to:
- Attempt to bypass security or access data you are not authorized to access
- Upload malicious code or interfere with the App's operation
- Use the App in a way that violates intellectual property or privacy rights
- Resell or sublicense the App except as permitted by the platform
6. Intellectual property
We retain ownership of the App, its code, branding, and documentation. You retain ownership of your store content and customer submission data stored in your store.
7. Availability and support
We strive to keep the App available but do not guarantee uninterrupted service. Support is provided on a reasonable-efforts basis at dmakvana33@gmail.com.
8. Disclaimer
THE APP IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.
9. Limitation of liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, WE WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, REVENUE, DATA, OR GOODWILL ARISING FROM YOUR USE OF THE APP.
10. Termination
You may stop using the App by uninstalling it. We may suspend or terminate access if you violate these Terms or if required for security or legal reasons. Provisions that by their nature should survive termination will survive.
11. Changes
We may update these Terms from time to time. Material changes will be reflected by updating the date at the top of this page. Continued use after changes constitutes acceptance.
